*Arizona Baptist Retirement Centers (ABRC) - Youngtown, Arizona (3 GREEN HOUSE® homes - operating)

One of the largest independent, non-profits in Arizona, ABRC has been providing quality, affordable care to elders for nearly thirty years, and currently serves over 600 residents in four Baptist Village communities located primarily in the Phoenix metropolitan area. Baptist Village communities offer residential living, assisted living, skilled nursing, and memory care with distinct personalized assistance at every level. ABRC currently is operating three assisted living Green House® homes at its Baptist Village, Youngtown campus. The Youngtown campus includes 160 independent living units, 44 assisted living units, and a 128-bed skilled nursing facility. The Green House® residences will replace the current assisted living facility. ABRC strongly believes that Green House® homes will help fulfill its mission to care for its elders with a compassionate heart, to help them recognize the value and purpose of their lives and to experience dignity, respect, and self-worth.  www.abrc.org/

*Asbury Park - Newton, Kansas (6 GREEN HOUSE® homes planned - 4 homes operating) 

Asbury Park, a continuing care retirement community in Newton, Kansas, is dedicated to the mission of supporting life and aging while enhancing physical, social, emotional and spiritual wellness through innovative programs and caring professionals. Success in that mission assures our elders the dignity and respect they deserve, plus the opportunity to continue living as productively and graciously as possible. Its 30-acre campus is home to over 200 residents, with 67 independent living cottages, 56 assisted living units and 89 nursing care beds, including an 11-bed memory support program for individuals with Alzheimer's and other dementia disorders. Asbury Park has a unique relationship with both the Kansas East and the Kansas West Conferences of the United Methodist Church, and also has been selected by the Kansas Department on Aging to partner with the state agency in bringing The Green House® Project to the state of Kansas. *Baptist Memorials Ministries - San Angelo, Texas (2 GREEN HOUSE® homes - operating)

Baptist Memorials Ministries (BMM) is a full-service retirement organization with a total of approximately 650 residents residing in homes, duplexes and apartments located in San Angelo and Burnet, Texas (independent living only).  BMM provides personal assistance services to retirees through its home health service (help with activities of daily living) and hospice care to people residing in an independent living or nursing facility.  As a faith-based organization, BMM provides a full-time chaplain on its campus.  In the initial phase of The Green House® development project, two houses, licensed as nursing facilities, were built adjacent to its 52-bed Alzheimer's facility.  A fundraising campaign supported the building of these two homes.  A second phase of development will replace two existing nursing facilities wings with six additional Green House® homes.  www.bapmem.com

*Buckner Retirement Services, Inc. (BRS) - Longview, Texas (2 GREEN HOUSE® homes - operating)

Buckner International was originally founded in Dallas in 1879 as Buckner Orphan’s Home to meet the needs of widows and orphans who were destitute, and was the first orphanage west of the Mississippi River. Buckner’s commitment to quality care with a Christian mission has endured the test of time and touched many lives. Today, BRS has 500 employees serving five retirement communities throughout Texas, with a total of 900 senior residents in independent living, assisted living, Alzheimer’s care, and skilled nursing homes.

BRS continues to lead Texas long-term care providers by implementing this new resident centered model.  The Holly House and The Hawthorne House at Buckner Westminster Place in Longview are the first Green House® homes in East Texas.  www.bucknerretirement.org/longview

*Calvary Baptist - Columbus, Georgia (1 GREEN HOUSE® home - operating)

For 15 years Calvary Baptist Church has been providing affordable housing at Calvary Community, and for four years The Gardens at Calvary has provided assisted living services. The Calvary Community Inc. Board has voted to expand their ministry to become a Continuing Care Retirement Community, to include a unit for Alzheimer residents and to seek a state license to provide skilled nursing home care. They will begin with one Green House® residence center and expand to meet the needs of the community. *Chelsea Jewish Nursing Home (CJNH) - Chelsea, Massachusetts (10 GREEN HOUSE homes in an urban  setting – 1 operating, with more opening soon) 

CJNH has been nurturing the community's frail elderly since 1919. CJNH provides skilled nursing and residential care for complications of aging, rehabilitation, Alzheimer's, dementia and end-of-life care. It is a modern, 123-bed residence with uncommon amenities in the heart of Chelsea, Massachusetts.

Chelsea's Leonard Florence Center for Living (LFC) is America's first urban Green House home, providing skilled nursing care for 100 residents in a warm, home environment. Two of homes are earmarked for residents with ALS (Lou Gherig’s disease) and multiple sclerosis and will offer pulmonary support and optimized mobility. The Center will complete the award-winning Miriam and Sheldon Adelson senior housing campus on Admirals Hill, complementing 69 units of affordable assisted living at the Cohen Florence Levine Estates and 36 studio apartments at the Florence and Chafetz Home for Specialized Care.

CJNH observes Jewish traditions, while meeting the spiritual needs of all residents. Devoted caregivers, some of whom have been employed for more than two decades, deliver outstanding personal assistance and nursing care. Govans Ecumenical Development Corporation (GEDCO) - Stadium Place - Baltimore, Maryland (4 GREEN HOUSE® homes, in an urban setting - in development)

GEDCO, based in Baltimore, Maryland, is an innovative, faith-based not-for-profit organization which has successfully developed a series of affordable housing projects with supportive services.  The GREEN HOUSE® at Stadium Place is an important component of GEDCO’s plan to develop a mixed-income, urban retirement community with a holistic continuum of care on the site of the former Memorial Stadium. The Stadium Place campus currently includes 340 apartments for low and moderate income older adults, as well as a YMCA, a community playground, and ThanksGiving Place, a meditative park.  Planning is also underway for market rate condominiums and a village center to serve the residents and the surrounding community.  www.gedco.org/GreenHouse

Green Hill, Inc. – West Orange, New Jersey (6 GREEN HOUSE® homes – in development) 

Green Hill, located in West Orange, New Jersey, has been providing senior living services since 1866, starting in Newark, New Jersey, with only 13 residents.  Now home to 148 residents, the community offers a continuum of care, providing skilled nursing services, sub-acute rehabilitation, hospice, assisted living and residential style living.  Future plans include replacing the 57 Medicare/Medicaid certified skilled nursing beds with six Green House® homes to be built on site.  Green Hill has been working diligently toward a more resident-centered culture; the addition of Green House® homes in the community will help complete those efforts. www.green-hill.com

Green House® Living for Sheridan - Sheridan, Wyoming (4 GREEN HOUSE® homes – in development) 

“Have you heard about The Green House Project?”  This question was first asked several years ago by a caregiver at the Sheridan Senior Center in Sheridan, Wyoming.  The Center later participated in the PBS documentary, "And Thou Shalt Honor," a poignant film illustrating the many aspects of caregiving.  Again, they were asked if they had heard of The Green House Project.  After observing the Center’s passion for improving the lives of elders, the filmmaker commented, “Sheridan would be a great place to get this started!”  Guided by an interdisciplinary team of community, industry, and government leaders, Green House® Living for Sheridan will build four homes on more than two acres of land, conveniently located one mile west of the Sheridan Memorial Hospital.  While they don’t anticipate breaking ground until 2008, this team has already broken significant ground in the legislative arena.

Jefferson County Nursing Home - Dandridge, Tennessee (3 GREEN HOUSE® homes - under construction)

Jefferson County Nursing Home is a county-government owned 135-bed dually certified skilled nursing facility located in Dandridge, Tennessee, at the foothills to the Great Smoky Mountains.  Opening in January, 1969, it has the reputation for being the "Crown Jewel" for the Jefferson County, Tennessee, government due to its cleanliness, quality nursing care and homelike environment.  The nursing home also prides itself on its food.  Residents in the main dining room experience "Restaurant Style Dining," as well as an early breakfast option.  The facility has 155 full- and part-time employees and offers intermediate and skilled care, as well as Physical, Occupational and Speech Therapies.  With the addition of three Green House® homes, the facility will grow to 160 beds.

*Lebanon Valley Brethren Home - Palmyra, Pennsylvania (4 GREEN HOUSE® homes - operating)

Located in Palmyra, PA, a central Pennsylvania town about 10 minutes from Hershey (the "Chocolate Capital" of the USA) Lebanon Valley Brethren Home (LVBH) is a freestanding Continuing Care Retirement Community founded in 1979.  LVBH is related to the Atlantic Northeast District of the Church of the Brethren, and is home to 575 residents, of whom 70% are in independent living apartments and cottages.  LVBH also offers skilled nursing, dementia care, personal care, adult day care, and an on-site child day care program.  The campus is currently built out on approximately 80 acres, with another 70 acres available for expansion.  LVBH operates four Green House® homes, the first in Pennsylvania, which replace two older skilled nursing units.  www.lvbh.org

*Mennonite Manor, Inc - South Hutchinson, Kansas (2 GREEN HOUSE® homes - operating)

Mennonite Manor was founded in 1973 and is affiliated with nine area Mennonite Congregations. Located in South Hutchinson, Kansas, its 170-acre campus offers the convenience of city living with the serenity of the country. Mennonite Manor is home to over 270 elders in a variety of living options, and is a leader in innovative senior care, offering the first Eden Alternative facility in Kansas. Mennonite Manor plans to replace the current 116-bed Medicare/Medicaid Certified Healthcare Facilities with Green House® homes over the next several years. Mennonite Memorial Home - Bluffton, Ohio (2 GREEN HOUSE® homes - in development)

Mennonite Memorial Home is a fully integrated retirement community comprised of two campuses that house 250 seniors in cottages, duplexes, independent apartments, assisted living, and nursing home facilities. In addition, MMH has a home health agency. The original campus is made up of 16 acres, surrounded by residential housing. The second campus, Maple Crest Senior Living Village, sits on 18 acres adjoining a residential community. Mennonite Memorial Home is situated in a lovely small town that enjoys the advantages of being the home of Bluffton University. Mennonite Memorial Home's goal has always been to provide an environment where older adults could live in a caring environment which ministers to spiritual needs and supports individual dignity. Mennonite Memorial Home is presently building two Green House® homes on its main campus for residents who require dementia care and skilled nursing. Mennonite Memorial is planning on replacing its existing nursing facilities with Green House® residences.  www.mmhliving.org

Mississippi Methodist Senior Services is the proud developer of the first Green House® residences in the United States. Mt. San Antonio Gardens - Pomona, CA (3 GREEN HOUSE® homes - in development)

Mt. San Antonio Gardens (MSAG) is a continuing care retirement community located in Pomona and Claremont, CA, and has been in operation since 1961.  Their 460 residents benefit from a comprehensive life care contract that guarantees residents will receive the care and services needed, even if they outlive their resources.  In addition to a variety of independent living homes, MSAG provides assisted living and skilled nursing on a 30-acre campus.  The development of three Green House® homes will further MSAG's mission of helping to ensure a high quality of life for older adults by providing comprehensive and secure services that promote wellness, independence, participation, and dignity.  www.the-gardens.org

Northwest Arkansas Senior Services / Legacy Village - Bentonville, Arkansas (6 GREEN HOUSE® homes planned - 3 operating)

The Green House® Homes at Legacy Village in Bentonville, Arkansas, are owned and operated by Northwest Arkansas Senior Services, Inc. (NWASSI), a local non-profit organization. NWASSI was formed in 2000 to develop a campus that would meet the diverse housing and service needs of seniors in our community. The first two Green House homes at Legacy Village opened in February of 2009. A total of six Green House homes are planned on the Legacy Village campus. Each home includes 10 apartments surrounding a common area core. All apartments are licensed as Level II assisted living units; 20 of the units will serve low-income seniors through the Medicaid Waiver program. When fully occupied, a team of over 45 persons will serve the elders through a system of self-managed work teams including trained Shahbazim, nurses, and administrative staff. At The Green House® Homes at Legacy Village, elders will be provided with every opportunity for growth and self-determination in an environment that protects, sustains, and nurtures.  www.legacyvillagearkansas.com

*Pinecrest Medical Care Facility - Powers, Michigan

Pinecrest is a 174-bed facility owned by Delta, Dickinson and Menominee Counties.  With over 220 staff members, Pinecrest provides a broad spectrum of high-quality services that respond to the physical, emotional, and social needs of residents.  Pinecrest Medical Care Facility has embraced the Eden Alternative, which complements its mission.  Pinecrest has built one Green House® home, but is planning to build a total of five, each housing 10 residents.  Pinecrest is licensed and certified with the Consumer and Industry Services of the State of Michigan, and is a member of the Health Care Association of Michigan and the Michigan County Medical Care facilities Council.  Pinecrest is a registered Eden Alternative Home. www.pinecrestmcf.org

*Porter Hills Presbyterian Village- Grand Rapids, Michigan (2 GREEN HOUSE® homes - operating) 

Porter Hills was established in 1970 at the Porter Hills Village campus in Grand Rapids Township. Today, Porter Hills provides retirement communities for nearly 800 older adults – at all income levels – on eight different campuses located throughout the greater Grand Rapids area. Within these communities, Porter Hills offers a variety of choices that include residential living, assisted living, or a full continuum of care with Alzheimer’s/dementia care and skilled nursing. Porter Hills is focused on providing its residents with amenities-filled communities and a complete sense of security by providing access to supportive care and services. Porter Hills offers a host of services to residents and West Michigan seniors, which include: physical, speech, and occupational therapies; skilled, in-home care following illness or injury; personalized, private duty services; and in-patient and outpatient rehabilitation care.  www.porterhills.org

*Presbyterian Villages of Michigan - The Village of Redford - Redford, Michigan (2 GREEN HOUSE® homes - operating)

A faith-based non-profit organization, Presbyterian Villages of Michigan (PVM) sponsors a growing number of retirement communities, senior housing facilities and programs for older persons. It offers a variety of retirement living and senior care options on eighteen villages, including independent living, assisted living and skilled nursing care to approximately two thousand individuals of all faiths. Its mission is guided by Christian compassion, concern and stewardship, and directs PVM to provide seniors with quality services, care and housing. PVM's vision is to maximize the quality of life for seniors in the state of Michigan. The Village of Redford offers a unique concept in senior living providing cottage apartments, assisted living, Alzheimer's memory care services, Medicare Rehabilitation Sub-Acute Services, and 24-hour skilled nursing care all on one campus. *Providence Seward Medical and Care Center - Seward, Alaska (4 GREEN HOUSE® homes - operating)

Providence Seward Medical and Care Center provides quality health care to residents and visitors with an extensive array of local services. Their team of physicians, nurses, and support staff deliver compassionate, quality care to all patients. Providence Seward has the experience and facilities necessary to diagnose and treat a wide range of medical conditions. Providence Wesley is a forty-bed, long-term care facility that provides skilled care and rehabilitation services. The center is staffed by registered nurses, licensed practical nurses, certified nursing assistants, physical, occupational, and speech therapists, and activity and dietary staff who work together to develop individual plans of care to help each resident attain the best possible quality of life. Providence Seward Medical and Care Center is building four Green House homes at Providence Seward Mountain Haven, with room for 40 additional residents.  This will be the first Green House project in Alaska. http://providence.org/alaska/seward/mthaven.htm

*Resthaven Care Community - Holland, Michigan (1 GREEN HOUSE® home - operating)
 

Resthaven Care Community's Green House® home, “The Vern & Lois Boersma Cottage,” is inspired by its mission: "To glorify God by providing a continuum of quality care and services that demonstrates the love and compassion of Jesus Christ.”  A non-profit mission, Resthaven constructed its first Green House® home on property near the campus of its existing skilled nursing facility, the Resthaven Care Center, on the south side of Holland.  The Vern & Lois Boersma Cottage houses 10 elders.  The people of Resthaven are dedicated to walking beside older adults offering honor and respect along with their skills in health care and in creating warm, welcoming communities.  Over 350 employees, 250 regular volunteers, and the prayer and financial support offered by the Holland area community allow Resthaven to stand beside its residents and provide benevolent care to those who are most frail and in greatest need, just as it has for over 60 years. www.resthaven.org

St. John's Home - Rochester, New York (15 GREEN HOUSE® homes - in development) 

St. John’s Home, part of St. John's Senior Communities, is a progressive long-term care community located in Rochester, New York. The organization has a long and distinguished heritage of serving the area for over 100 years. St. John's Home, a 475-bed urban provider, is the only registered Eden Alternative nursing home in Rochester.  Central to St. John’s is the Eden mission to nurture life for elders with vibrant, caring, life-affirming relationships and to create a resident-centered community. St. John's offers many levels of quality care to match the unique needs of each individual. Premier services include skilled nursing care, Alzheimer's/Dementia care, transitional care, rehabilitation, adult day services, and hospice care. St. John’s Home will build on its successful experience with the Eden philosophy by developing 15 new Green House® homes in the Greater Rochester Community and then transforming the existing structure to create 32 new Green House® homes within.  www.stjohnshome.com

St. John's Lutheran Ministries is a community of diverse people sharing God's healing presence, with a mission to provide living opportunities within nurturing environments of hope, dignity, and love. A social ministry of the Evangelical Lutheran Church in America, St. John’s was incorporated in 1960 and is owned by 22 ELCA congregations and governed by a 13 person Board of Directors.

Started as the first HUD-financed senior retirement community in Montana, St. John’s has grown to become the largest senior campus in the state, consisting of 109 low-income retirement apartments, 186 skilled nursing beds, an adult day care center, and a rehabilitation center. In partnership with St. Vincent Healthcare, the largest hospital system in Montana, St. John’s also operates the first full service Continuing Care Retirement Community in Montana, consisting of 122 independent living and 59 assisted living apartments. In addition to services for elders, St. John’s also provides on-site childcare through the Center for Generations.

The assisted living Cottages at St. John's, modeled after The Green House® approach, opened in August 2005.  Five skilled-nursing-level Green House® homes currently are operating on their campus.  A future phase of five additional skilled nursing Green House® homes is envisioned to complete a full transition of nursing care services to The Green House® model on the St. John’s campus. www.sjlm.org

St. Martin's in the Pines is a Continuing Care Retirement Community providing residential care to approximately three hundred and fifty elders. It is owned and operated by the Episcopal Foundation of Jefferson County founded in 1955 with the mission to assist the aging. Today, St. Martin's consists of a 138-bed skilled nursing facility, a 71-bed assisted living facility, a 35-bed dementia care facility, and a 100-unit independent living apartment building. As part of a long-range plan, the leadership has identified the need to replace the skilled nursing facility with a care model that will lead to better care, and better quality of life, for the elders living there, as well as increased elder and employee satisfaction. Located in Magnolia, Arkansas, Wentworth Place and its predecessor companies have continuously served the residents of Columbia County and southwest Arkansas for over fifty-years.  Licensed for 113 residents, Wentworth Place is dedicated to providing quality care and improving the quality of life for all of our residents in a warm, comfortable, respectful environment.  Our staff of over seventy interdisciplinary healthcare professionals is dedicated to providing real culture change in a team-oriented environment.  Wentworth Place is a skilled nursing facility that provides care for geriatric and chronically ill elders as well as intensive short-term therapy for those transitioning from hospital to home.  Our dedication to individualized elder care will be further enhanced by the relocation from our current facility into our new, “park-like” ten-acre campus in Magnolia, which will initially contain five (5) Green House homes and a 40-bed rehabilitation pavilion.

For over 35 years, Sunrise has earned its reputation as the premiere not-for-profit organization dedicated to providing the support and assistance necessary so that people with developmental disabilities can take their rightful place in the community.

Sunrise recognizes that all people want and have a right to a full and productive life, which includes a home, family, work, and participation in community groups and activities. Sunrise is committed to ensuring that individualized support and services are designed and provided to build on people's strengths, interests and needs.

Sunrise offers a broad range of services for people with disabilities and has expanded its organization to include more than 12 not-for-profit agencies in five states. Sunrise presently serves over 2,200 individuals and employs more than 4,000 people. Sunrise will be adding four Green House® homes to their campus in Miami.

Since 1886, Tabitha Health Care Services has identified community health needs and responded by providing compassionate, professional services. Tabitha has successfully pioneered such essential community programs as Home Health, Hospice, Meals on Wheels, and intergenerational day services. The organization's continuum also includes HUD housing for independent living, specialized Alzheimer’s programs, rehabilitation and long-term care. Tabitha is committed to meeting the physical, spiritual and social needs of the elders in the community, and cherishes its role as an advocate for improving existing elder care models. *The Cove - Jamie's Place - Winthrop, Washington (2 GREEN HOUSE® homes - operating)

The Cove began as a food bank to serve the nutritional needs of those living in poverty in the Methow Valley, in Washington. Soon, The Cove began providing emergency financial assistance to families in temporary crisis, and found that individuals and families with special circumstances needed more than emergency assistance. The Cove continues to evolve to serve the needs of its community. The “Guardian Angel Program” is The Cove’s newest program, and is designed to provide direct support to those who are chronically in need of help by having members of the community “adopt “ a person or family. The Cove’s long-term vision for the future is to provide opportunities for low-income families to purchase their own homes. The Green House® model is a great fit for The Cove because it supports the vision of providing affordable housing to members of the local community. The Cove is a 501c3 and accepts donations. Other funding sources include grants from private and public foundations and programs. The Cove depends on a core of volunteers and always welcomes more help.  www.thecovecares.com

Founded on a commitment to superior residential healthcare for individuals who need skilled nursing care, The Eddy was established in 1928 in Troy, New York, as a 19-bed nursing home. Today, The Eddy is an affiliate of Northeast Health, a comprehensive continuum of healthcare, supportive housing and community services that reaches 15 counties and serves more than 19,000 people every year. The Eddy’s services help maximize independence, quality of life and dignity of elderly individuals. Green House® development is the next step in fulfilling that mission. Virginia Mennonite Retirement Community (VMRC) - Harrisonburg, Virginia (10 GREEN HOUSE® homes - in development)  

Founded in 1954, Virginia Mennonite Retirement Community is a continuing care retirement community serving senior adults of all faiths. VMRC is located in Harrisonburg, VA, close to Eastern Mennonite University and James Madison University. VMRC offers an enriching retirement experience which includes townhouse living, apartment homes, HUD apartments, assisted living and skilled nursing care for nearly 750 elders. Campus amenities include a fully-equipped wellness center, indoor pool, onsite rehabilitation program, walking trails and educational activities. 

VMRC is committed to being a leading resource in providing quality homes and services that people want and need to age well. The plans for VMRC’s GREEN HOUSE® project include total replacement of a 120-bed nursing facility to 10 Green House® homes in two campaign phases.
